Position Summary
The Physical Therapist will evaluate and treat patients in an outpatient setting, develop individualized plans of care, document thoroughly and efficiently, and communicate effectively with patients, providers, employers, and referral sources as needed. The ideal candidate is confident, compassionate, dependable, and excited about working in a growing private practice.
Duties
- Perform physical therapy evaluations and re-evaluations
- Develop and implement individualized treatment plans
- Provide high-quality, evidence-based patient care
- Document evaluations, daily treatment notes, progress notes, and discharge summaries in a timely manner
- Educate patients on diagnosis, prognosis, home exercise programs, and plan of care
- Progress patients appropriately based on goals and functional presentation
- Communicate professionally with physicians, case managers, employers, and other healthcare providers as needed
- Maintain compliance with clinic policies, state practice standards, and payer requirements
- Contribute to a positive clinic culture and excellent patient experience
Requirements
- Graduate of an accredited Physical Therapy program
- Licensed or eligible for licensure as a Physical Therapist in Oregon
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Excellent clinical reasoning and documentation skills
- Desire to grow within a small, fast-paced private practice

Pay: $42.00 - $53.00 per hour
