Registered Nurse (RN) – Per Diem Home Visits

At Pivot Point Home Care, we're committed to delivering high-quality, in-home nursing care with compassion and professionalism. We're currently seeking a Per Diem RN to support clients in Rehoboth and the surrounding areas, offering flexibility and meaningful patient relationships.
What You'll Be Doing:
- Conduct skilled nursing visits and assess patients in their homes
- Create and update customized care plans based on patient needs
- Complete medical reviews, home safety checks, and physical assessments
- Monitor patient conditions and communicate changes to the care team
- Coordinate services to ensure cohesive, patient-centered care
- Maintain accurate, timely documentation of visits and progress
Who We're Looking For:
- Licensed Registered Nurse in Massachusetts (in good standing)
- Graduate of an accredited nursing program
- 1 year of clinical nursing experience (home care preferred)
- CPR certified
- Strong clinical judgment and communication skills
- Able to work independently and manage a flexible schedule
