Summary:

The PHSP Coordinator supports the implementation and coordination of the Partnership HealthPlan of California (PHC) Perinatal Services (PHSP) and AIMSS programs under the guidance of the Outreach RN and Chief Medical Officer. Working collaboratively with a multidisciplinary team—including the Clinical Director, RNs, LVNs, and CHR/CHWs—the Coordinator ensures perinatal clients receive comprehensive services across nutrition, health education, behavioral health, and case management during and after pregnancy. Location is Burney OR Alturas clinic.

Responsibilities:

1. Deliver perinatal services in accordance with PHC protocols, ACOG guidelines, and safety/screening standards.

2. Utilize PHC billing codes and allowances to support accurate documentation and reimbursement.

3. Assist in case management, including intake assessments, care plan development, documentation, and participation in case review meetings.

4. Provide health education through home visits, group sessions, and telehealth, as appropriate.

5. Support client access to services, completing referrals and coordinating care across clinical and community-based resources.

6. Collaborate with the Outreach RN to implement the Family Spirit Program, delivering lessons and support for caregivers of children from birth to age 3.

7. Assist with data collection, EHR documentation, and report generation (e.g., billing summaries, coding reports).

8. Monitor and support the achievement of AIMSS Work Plan goals.

9. Identify and outreach to eligible families within the service area to engage them in available programs.

10. Serve as a liaison between local health providers and Native American communities, including performing follow-up home visits when needed.

11. Support equipment and supply coordination, including health and safety items (e.g., car seats, fire extinguishers, N95 masks, air purifiers).

12. Provide occasional support with CHR/CHW responsibilities when needed.

13. Maintain communication with healthcare providers regarding patient concerns or follow-up needs.

14. Participate in training to expand knowledge in nutrition, behavioral health, prenatal/postpartum care, and lactation education.

15. Perform other duties as assigned by the supervisor.

Qualifications:

1. High school diploma or equivalent.

2. Associate degree in health-related field, preferred.

3. 2–5 years of experience in:

· Maternal and child health programs

· Case management or health education

· Community outreach or public health

4. Knowledge of perinatal care, nutrition, behavioral health, and postpartum support.

5. Have prior CHR training, with a minimum of 5 years of field experience.

6. Family Spirit Educator certified, or may be completed within initial 6 months of hire.

7. Working knowledge of basic medical procedures and terminology.

8. Strong customer service, communication, and interpersonal skills; ability to build rapport with clients and families.

9. Ability to work both independently and as part of a multidisciplinary team.

10. Proficiency with EHR systems, data entry and basic reporting.

11. Ability to conduct home visits, complete assessments, and develop care plans.

12. Valid, unrestricted driver’s license.

13. Current CPR certification.

14. Willingness and ability to travel for training and program delivery.

15. Successful completion of a pre-employment drug screening.

16. Cultural sensitivity to Native American and American Indian communities.

17. Preference will be given to Native American applicants in accordance with P.L. 93-638.
