Job Overview:
The CHR/W Program Coordinator is a level above a CHR/W. This position assists with oversight of the CHR/W Alturas team and serves as back-up for the CHR/W Program Supervisor. The CHR/W Program Coordinator assures that the CHR/W teams’ workload, services and CHR/W guidelines are in line with our program scope of work. The Outreach Director will provide direct day to day onsite supervision, while the CHR/W Program Supervisor will provide program related oversight and program engagement to this position. This position provides knowledge and skills as a seasoned CHR/W who has enhanced knowledge of the CHR and Community Health Worker. The position continues to function as a field healthcare worker. With understanding that the CHR/W position combines the traditional CHR role with that of the CHW. This enables the worker to maintain the enhanced services that are offered to our Native members while being able to cross over to non-native allowable CHW services. Location is at Burney OR Alturas Clinic.

Specific Duties and Responsibilities:

1. Prepares Case Management documentation for the Outreach Director and CHR case reviews. Participate in Case Management meetings/reviews.

2. The CHR/W Coordinator clearly understands the specific duties and responsibilities that differentiate between the CHR and CHW as outlined in those Job Descriptions. Assist the Director in assuring that CHR/W’s are following the CHR/W Standards of Practice and Scope of Work for CHR/W workers Understanding the services provided to CHR eligible and CHW services.

3. Provide oversight and maintain documentation of CHR/W’s required training both new training and all required ongoing training.

4. Coordinate with the Outreach Director and CHR/W Supervisor on CHR client allocation, staffing and CHR/W’s Request for Services follow-up.

5. CHR/W Coordinator will assist in equipment, supplies and resource purchases and tracking.

6. Collect, gather and assure CHR/W reports are completed correctly and timely. When needed provide oversight and guidance to CHR’s regarding these reports. Reports may include; weekly agenda, end of month report, mileage logs, time card and EHR documentation.

7. Pursues the goals and objectives of the Community Health Representative (CHR) and CHW programs.

8. Identifies and locates Native American families in the service delivery area to provide services.

9. Utilizes the electronic programs to document and data entry.

10. Completes periodic home visits to identify health and social needs of patients and assists with referrals and access to care.

11. Provides basic health care education and instructions as outlined in the CHR Competency Checklist and CHR modules.

12. Serves as a liaison between local health providers and the Native American Community. CHRs may be referred by PRHS Medical, Dental, PRC (Purchased & Referred Care) to make a home visit for an identified healthcare need such as checking vital signs, delivering supplies, and delivering a message to a client when a department has tried and failed to reach a patient by phone or mail for an important reason.

13. CHW services require a written recommendation submitted to the Manage Care Plan (Partnership HealthPlan of California) by a licensed practitioner who can recommend CHR/CHW services within their scope of practice include; physician assistants, nurse practitioners, clinical nurse specialists, podiatrists, nurse midwives, licensed midwives, registered nurses, public health nurses, psychologists, licensed marriage and family therapists, licensed clinical social workers, licensed professional clinical counselors, dentists, registered dental hygienists, licensed educational psychologists, licensed vocational nurses, and pharmacists. *Service Recommendation Form CHW.

14. Supports access and provides information to clients as needed for completion of paperwork for services.

15. Provides to native client’s provides safety equipment, i.e. car seats, fire extinguishers, smoke and carbon monoxide alarms; assists elders with installation as necessary. Assistance may be requested with acquiring and providing N95 masks and HEPA air purifier machines and filters during events such as wildfires that cause poor air quality.

16. May provide to native client’s storage containers for water or water bottles in cases of water disruption at client’s home.

17. Documents and maintains health records on clients in accordance with appropriate laws and regulations.

18. Communicate with providers or health care facilities to report concerns regarding patient status.

19. For native clients: When the Transportation Dept. is unable to provide transportation for a client. CHR may occasionally provide transportation needs of local clients to and from clinic appointments (medical and dental) and referrals to outside providers.

20. For native clients- picks up and delivers prescriptions from local pharmacies to clients; may not pick up and deliver prescriptions that are controlled substances without proper authorization and signatures.

21. Provide Family Spirit and Perinatal Health education: Conduct and provide personal lessons for parents, family and caregivers regarding prenatal, infant and child care lessons for infants and children from birth to 3 years of age. This program is supervised by the Outreach RN trained in the Family Spirit Program and American Indian Maternal Support Services (AIMSS) Program.

22. Other duties as assigned by supervisor.

Qualifications:

1. High school diploma or equivalent- Associate Degree preferred

2. CHR and CHW Trained and at least 5 years of CHR field work.

3. Possess a valid Drivers License & current car insurance.

4. Possess a current CPR card.

5. Must be willing to submit to and pass a pre-employment drug screening test.

6. Sensitive to the native American and American Indian community, culture, and traditions.

7. Knowledge of basic medical procedures and terminology.

8. Must be customer service oriented.

9. Be able to communicate and establish rapport with clients and families.

10. Preference will be given to Native Americans in accordance with P.L. 93-638.

Pay: $21.54 - $23.94 per hour

Expected hours: 40.0 per week
