Supportive Service Coordinator
Provide supportive services in a way to empower and enhance resident independence to include monitoring of services provided and follow-up communication with service providers.
Plan, promote, and implement at least one health and wellness program per month, contribute to the Property’s monthly newsletter, along with weekly notice distributed to every resident.
Maintain confidential, individual files on residents in compliance with HUD which will contain at least: intake/ assessment information, confidentiality statement, and any copies of needed documents.
Develop and sustain partnerships with the Area Agency on Aging, the Aging and Disability Resource Connection, community-based supportive service providers and other community stakeholders.
Maintain an up-to-date resource manual for resident and staff use.
Adhere to the HUD Service Coordinator in Multi-Family Housing Program Resource Guide and ensure compliance within outlined HUD Service Coordinator requirements.
The ideal candidate will possess the following:
Bachelor’s Degree and/ or equivalent combination of work experience in Social Sciences.
Experience with referral processes, advocating, and problem-solving for the elderly and people with disabilities.
Exceptional communication and interpersonal skills with the ability to network and establish relationships within the community and Macomb County.
Ability to work independently and in a team environment while being flexible and receptive to a diverse population.
Strong computer and technical skills.
Job Type: Full-time
Pay: $36,000.00 - $48,000.00 per year
