What are the responsibilities and job description for the MDS Coordinator position at Pioneer Valley Living and Rehab?
Pioneer Valley Living and Rehabilitation is a skilled nursing facility located in Sergeant Bluff. We are looking for a new team member.
The MDS Coordinator is a critical member of the health care team responsible for all aspects of the MDS process, including care planning and meeting with families regarding the care of their loved one.
Responsibilities:
- Coordinate the development, completion and transmission of assessments in accordance with the regulatory requirements
- Review and manage reimbursement
- Manage care plans, including rehabilitative programs
- Assist with admission and discharge planning/coordination
Qualifications:
- MDS and/or Restorative Nursing experience preferred
- Valid Iowa RN license
- Knowledge of clinical standards of practice, regulations and reimbursement governing long-term care and Medicare participation
