Physical Therapist (PT) / Physical Therapist Assistant (PTA)
Location: Rocky Ford, CO
Status: Full-Time / Part-Time / PRN
Pay: $35-$55 (dependent on credentials and experience)

We’re growing our therapy team at Pioneer Health Care Center and have an opportunity for a Physical Therapist (PT) and Physical Therapist Assistant (PTA) to join us!
Pioneer offers both inpatient and outpatient therapy services, giving our therapy team the opportunity to work with a variety of patients and residents with different needs and goals.
What You’ll Do
Your responsibilities will be based on your license and scope of practice, but may include:
- Provide individualized physical therapy services based on patient and resident needs and established goals
- Work on mobility, strength, balance, gait, transfers and functional independence
- Provide therapy services across inpatient and outpatient settings based on caseload and facility needs
- Monitor progress and response to treatment
- Communicate closely with the therapy and interdisciplinary teams
- Educate patients, residents and caregivers as appropriate
- Complete clinical documentation accurately and timely
- Participate in care planning and discharge planning
For PTs: Complete evaluations and reassessments, establish and update plans of care, set functional goals and provide clinical direction within the physical therapy program.
For PTAs: Provide treatment under the direction of the Physical Therapist, carry out established plans of care and communicate progress and changes with the supervising PT.
What We’re Looking For
- Current Colorado Physical Therapist or Physical Therapist Assistant license
- Graduate of an accredited PT or PTA program
- Strong communication and documentation skills
- Ability to work collaboratively with the therapy and interdisciplinary teams
- Previous rehabilitation experience preferred, but not required
