POSITION SUMMARY
The Clinical Case Manager is responsible for carrying out a structured, individualized and closely
supervised living/training experience designed to promote the acquisition of independent living skills,
community resources utilization and self-sufficiency on the part of group home resident.

Essential Functions Statement(s)
- Meet production standards consistent with your role, which is 60%. * Complete all Rule 140 Medicaid documentation concurrently, which is defined by end of shift worked or within 24 hours of service delivery. Trains and Assists Program Techs and Case Managers in appropriate Medicaid Documentation as well Assessments in Electronic Medical Record Systems (EVOLV currently) * Observes assigned program participants' behaviors and interactions, affirming and/or correcting such behaviors accordingly. * Performs activities, including peer engagement, resource development, and socialization activities, in a safe, effective, respectful and healthful manner. Facilitates Groups weekly; including House meetings at Independent Homes * Engage clients in regards to their Integrated Assessment and Treatment Planning and ensures IATPs, LOCUS, and DLA-20 Assessments are completed every six months for clients * Link clients to resources within in the community. * Problem-solve and advocates client issues regarding treatment, appointments, relationships with community, agency and family * Provides coordination of services and make appropriate referrals to collateral providers.
- Establishes and fosters communications and positive interactions among staff, participants, families, and collaterals. * Works collaboratively with staff members on accomplishment of Strategic Objectives and activities and maintains clinical certification. * Working as part of a team of individuals involved in provision of services, willingly and openly shares information with team members. * Performs all other duties as assigned by Clinical Manager

SKILLS & ABILITIES
Education:
Bachelor's Degree (four year college or technical school) Required, Field of Study: Human Services. Mental Health Professional Required
Experience:
1 plus years of experience in related position Prior experience working in a social service setting is a plus.
Computer Skills: Microsoft Word, Evolv, Outlook
Certifications & Licenses: Certified Trauma Professional (CTP)
Other Requirements:
Must have and maintain a valid driver's license. Must be able to meet agency driver safety requirement of carrying personal vehicle insurance with the following limits: $50,000 bodily injury, each person $100,000 bodily injury, each accident $50,000 property damage
