Home Health and Hospice Registered Nurse (RN)

Overview

We’re hiring a compassionate RN to provide home health and hospice care to adults and seniors in their homes. In this role, you’ll complete assessments, create care plans, provide skilled nursing visits, and support patients and families with education, coordination, and comfort-focused care.

Position Highlights:

• Complete initial and ongoing assessments

• Develop and update individualized care plans

• Provide skilled nursing visits, medication administration, and patient education

• Collaborate closely with physicians and the care team

• Support patients and families with symptom management and end-of-life care

• Act as RN Case Manager when assigned

• Participate in interdisciplinary team meetings

• Occasional on-call rotation available with additional compensation(Weekdays 5pm–8:30am and 1 weekend/month)

Requirements:

• Active RN license in the state

• Minimum 1 year of adult or geriatric medical-surgical nursing experience

• U.S. Citizen or Green Card holder
