- Perform comprehensive wound and skin assessments
- Develop, implement, and manage individualized wound care plans
- Administer treatments and ensure accurate, timely documentation
- Monitor healing progress and adjust care plans with physicians
- Track and report pressure ulcers and skin integrity data
- Lead prevention programs (Braden Scale, risk interventions)
- Educate and mentor nursing staff on best practices in wound care
- Ensure compliance with state, federal, and facility standards
- Support Quality Assurance & Performance Improvement (QAPI) initiatives
- Maintain proper wound care supplies and resources
- Active RN or LPN/LVN license (Missouri)
- At least 1 year of wound care or treatment experience in long-term care
- Preferred: 2 years in Skilled Nursing Facility (SNF)
- Current BLS/CPR certification
- Strong clinical judgment and documentation skills
- Passion for resident-centered care and outcomes
- Experience with pressure ulcer prevention and healing protocols
- Strong knowledge of skin integrity management
- Leadership mindset with a willingness to train and support staff
