Job Summary:
We are seeking a compassionate and organized In-Home Care Targeted Case Manager to provide personalized support and guidance to individuals in need of home-based healthcare services. The ideal candidate will work closely with clients, their families, and Pinnacle In-Home Care staff members to ensure the delivery of appropriate and high-quality care. Provides rotating on-call scheduling service.
Responsibilities:
1. Assessment and Care Planning: Conduct comprehensive assessments to evaluate the client's medical, social, and psychological needs. Develop individualized care plans that address these needs while considering the client's preferences and goals.
2. Client Advocacy: Act as an advocate for clients, ensuring their needs and preferences are communicated effectively to healthcare providers. Support clients in navigating the healthcare system.
3. Coordination of Services: Collaborate with healthcare professionals, including nurses, therapists, and social workers, to coordinate the delivery of in-home care services. Ensure that care plans are followed and adjusted as needed.
4. Regular Monitoring: Continuously monitor the client's progress and adjust the care plan as necessary. Provide ongoing support to clients and their families.
5. Crisis Intervention: Respond to emergencies or crises that may arise within the home care setting. Implement appropriate interventions and connect with emergency services when required.
6. Documentation: Maintain detailed and accurate records of client assessments, care plans, progress, and any significant changes. Ensure compliance with all documentation and reporting requirements.
7. Education and Support: Educate clients and their families on available resources, health conditions, and self-care practices. Provide emotional support and guidance to promote overall well-being.
8. Community Outreach: Establish and maintain relationships with community resources and service providers that can benefit clients. Assist clients in accessing community-based services when appropriate.
9. Compliance and Regulations: Stay up to date with relevant regulations, guidelines, and best practices in in-home care. Ensure that all services provided meet legal and ethical standards.
Physical Requirements:
1. Prolonged periods of sitting at a desk and working at a computer.
2. Must be able to lift 15 pounds at times.
Qualifications:
- LPN or Master of Social Work or a four-year degree. Must be licensed in the State of Arkansas as a Registered Nurse, Licensed Practical Nurse, Social Worker or have a bachelor’s degree from an accredited institution or have two (2) years’ experience providing case management services to adults with physical disabilities.
