Responsibilities
The RN Case Manager is a vital member of the interdisciplinary team, and is a primary source for healthcare, comfort and social services for patients and families . As such, patience, empathy and flexibility are equally critical to success in this role. Responsibilities include:
- Providing nursing care to patients as prescribed by the physician and/or approved care plan
- Carrying out ongoing assessments and evaluations of patients’ conditions to ensure proper care is being given and revising plans of care, as needed
- Medicating and treating patients as prescribed by the physician and/or approved care plan
- Initiating and applying preventative and therapeutic nursing procedures and techniques including, if qualified, specialized procedures , then communicating those courses of action to the physician, interdisciplinary team members and families
- Completing and maintaining accurate, relevant clinical notes regarding patient care and observed conditions
- Relaying changes in conditions, circumstances and needs to the interdisciplinary team, physician, families and caregivers to determine appropriate adjustments to existing care plans
- Aiding patients in daily living tasks to maximize comfort and enable increased self-sufficiency
- Assisting patients and families in appropriate continuity of care plans, in-home and in the facility
- Supervising ancillary personnel and delegates, as needed
- Willingness to travel in and around the Pittsburgh area
Qualifications
The RN Case Manager is expected to have excellent observation skills, good nursing judgment and topnotch written and oral communication abilities. This role requires a strong self-starter with the ability to thrive in both a team and independent environment. All candidates are expected to be graduates of accredited schools of professional nursing, and have current RN licenses through the State Board of Nursing. One year of RN experience is required, with specific hospice, medical, surgical or critical care work preferred.
Since some travel to patients’ homes is required, a valid driver’s license and reliable mode of transportation is also mandatory.
