Responsibilities:
- Assess, diagnose, and treat residents according to your scope of practice and NYS Law
- Take and review patients' medical histories, conduct physical examinations, and write admission and follow-up notes in the EMR
- Order and interpret labs and diagnostic studies, diagnose and treat medical conditions, and make referrals when needed
- Prescribe medications, including controlled medications
- Participate in the development and implementation of individualized care plans
- Collaborate with the interdisciplinary team to provide comprehensive care to residents
- Current nursing and advanced practice license in compliance with State law
- Current DEA licensure
- Ability to develop and maintain collaborative relationships
- Experience in working with older adults with complex medical conditions
- Ability to assess subtle changes in health conditions of the older adult
- Working knowledge of State and Federal Long Term Care regulations
- Excellent administrative, organizational, and verbal skills
- Effective communication skills with the geriatric population
- Ability to work independently
- Detail oriented
- Dependable and reliable
- Proficiency in EMR systems (Sigma/MatrixCare a Plus)
- Clinical background in adult, family, or geriatric practice
- Past experience working in a nursing home or with older adults in other settings
- Experience in working in a post-acute setting
- Every Wednesday
- $50 - $80 HOURLY
