Job Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse to join our team at Swan Lake Nursing & Rehabilitation in Patchogue, New York. Responsibilities:
- Provide a full range of healthcare services, including assessment and treatment of common acute and chronic health problems, in accordance with current standards of nursing practice
- Supervise, coordinate, and evaluate the health care team and services on the unit
- Collaborate with other healthcare professionals to develop and implement individualized care plans
- Provide guidance and support to subordinate personnel
- Maintain accurate and detailed records of patient care
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
- Current licensure as a Licensed Practical Nurse in the State of New York
- Graduate of an approved NLN accredited nursing program or State Approved Vocational Program
- Ability to provide leadership and supervision to subordinate personnel
- Genuine interest in gerontology and an understanding of the special needs of the geriatric elder
- Successful completion of pre-employment health screening and background check
- CPR certification
- $38 - $40 HOURLY salary range
