Pinnacle Care Providers is seeking a compassionate, patient-focused PRN Physician (MD/DO) to join our growing team serving Skilled Nursing Facilities, Assisted Living, and Independent Living communities throughout the Springfield and Dayton, Ohio region.

Responsibilities
· Provide comprehensive medical care to geriatric patients in post-acute and long-term care settings.
· Evaluate, diagnose, and manage acute and chronic medical conditions.
· Collaborate with advanced practice providers, nursing staff, care coordinators, and facility leadership.
· Develop individualized treatment plans that promote quality of life and reduce unnecessary hospitalizations.
· Document patient encounters accurately and efficiently within the electronic medical record.
· Participate in interdisciplinary care planning to ensure exceptional patient outcomes.
· Minimum 2 days per week.
Qualifications
· MD or DO degree
· Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
· Active Ohio medical license (or ability to obtain)
· DEA registration
· Strong communication and collaboration skills
· Experience in skilled nursing, post-acute, or geriatric medicine is preferred but not required
