As a Care Coordinator, you’ll work side-by-side with our Physicians and Nurse Practitioners in skilled nursing, long-term care, assisted living, and independent living facilities. You’ll play a vital role in ensuring our patients receive the highest quality care.

What You’ll Do:
- Be the bridge between providers, facilities, patients, and families.
- Coordinate patient care and organize census data.
- Pre-round with providers, gathering and presenting key medical information.
- Scribe patient encounters and ensure accurate EMR documentation.
- Track visits and progress notes to maintain consistent care.
- Bill patient visits accurately and on time.
