Qualifications :
-
Graduate of an accredited Speech Therapy program
-
Licensed Speech Therapist in the state of Florida
-
Minimum of two years of clinical experience in speech therapy (home health preferred)
-
Current BLS Certification
-
Valid driver’s license and access to a reliable, insured vehicle
-
Strong communication, observation, and critical thinking skills
-
Ability to work independently with excellent time management
Speech Therapist Position Overview:
The Speech Therapist (ST) is responsible for providing skilled speech-language pathology services to patients in their homes. This includes assessing, diagnosing, and treating speech, language, cognitive, and swallowing disorders to help patients regain essential communication and functional abilities. The ST works under physician direction and as part of a collaborative care team.
Key Responsibilities:

Conduct initial and ongoing patient assessments, including OASIS evaluations

Develop and implement individualized treatment plans based on physician orders

Deliver therapy interventions to improve speech, language, voice, cognition, and swallowing
-
Educate patients and families on therapy techniques and expectations
-
Collaborate with physicians, nurses, and interdisciplinary team members
-
Document patient progress and treatment outcomes accurately and timely
-
Supervise Home Health Aides when speech therapy is the primary discipline
-
Participate in care plan updates and ensure alignment with physician directives
