Summary
We are seeking experienced Referrals Coordinator for Primary Care to join our Care Teams. This vital role involves managing patient referrals and ensuring seamless communication between providers to support our mission of delivering exceptional healthcare services. The ideal candidate will be detail-oriented and experienced in medical office procedures with good knowledge of value-based medicine, medical records handling and electronic medical records.
Responsibilities
- Coordinate and process patient referrals efficiently to ensure timely access to specialist services.
- Communicate effectively with healthcare providers, insurance companies, and patients.
- Maintain accurate medical records and referral documentation.
- Verify insurance coverage and manage insurance-related inquiries.
- Ensure compliance with HIPAA regulations and confidentiality standards.
- Support the medical team by scheduling appointments and managing patient information.
- Assist in the preparation of referral reports and follow-up documentation.
- Assist patients in problem solving potential issues related to the health care system, financial or social barriers (e.g. arranging for interpreters as appropriate, transportation services, medical leave forms, or prescription assistance).
- Be the system navigator and point of contact for patients and/or caregivers assuming an advocate role on the patient's behalf to ensure approval of medically needed supplies or services as prescribe by the medical providers.
- Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providers and specialists.
- Contribute to a positive patient experience through professional communication and organization.
Qualifications
- Proven experience in medical office administration or healthcare coordination with a minimum 3 years' experience.
- Strong knowledge of managed care, insurance verification/ authorizations, medical coding (CPT, ICD-10) and medical terminology. Billing knowledge a plus.
- Familiarity with medical records handling and release of information following HIPAA regulations.
- Must have experience with electronic medical records (ECW preferably) and good computer skills with Microsoft applications.
- Excellent organizational, multi-tasking and fast learning skills with attention to detail. Team player a must.
- Ability to communicate effectively with patients, providers, and insurance representatives in order to provide a good customer and patient experience.
- Office experience in a healthcare setting is preferred.
- Spanish speaking preferred.
