Position Summary
The Social Services Assistant / Admissions Coordinator is responsible for assisting residents and families with psychosocial support, discharge planning, and community resources, while also managing the admission process to ensure smooth and welcoming transitions into the facility. This dual role requires strong organizational, communication, and customer service skills to support residents, families, referral sources, and the interdisciplinary care team.
Essential Duties and Responsibilities
Social Services Functions:
Assist with psychosocial assessments and care plans for residents.
Support residents and families with adjustment to facility life.
Advocate for resident rights and help resolve concerns or grievances.
Participate in care plan meetings and provide input on social/emotional needs.
Assist with discharge planning, coordinating resources and services.
Document services provided in compliance with regulations.
Admissions Functions:
Serve as the primary contact for new referrals and admissions.
Conduct facility tours and provide information to prospective residents and families.
Coordinate with hospital case managers, physicians, and referral sources.
Collect and review admission paperwork, insurance verification, and clinical documentation.
Ensure new residents have a smooth admission experience, including room preparation and introductions to staff.
Track census and admissions data as required.
Qualifications
High School diploma or equivalent (Associate or Bachelor’s degree in Social Work, Human Services, or related field preferred).
Experience in long-term care, admissions, or social services strongly preferred.
Knowledge of Medicare/Medicaid, insurance verification, and skilled nursing regulations a plus.
Strong interpersonal, communication, and organizational skills.
Ability to work collaboratively with residents, families, and staff.
Proficient in Microsoft Office and electronic medical records (EMR).
Physical Demands
Must be able to sit, stand, bend, and lift up to 25 lbs occasionally.
Ability to move throughout the facility and visit resident rooms as needed.
Work Environment
Skilled Nursing Facility setting with frequent interaction with residents, families, and healthcare professionals.
May require occasional evening or weekend availability for admissions.
