What are the responsibilities and job description for the Individual Support Worker position at PINELAND BH/DD CSB?
This position is to work for Community Housing offering services at one residential centers in Jesup, GA (you have to have a flexible schedule, we are hiring for all shifts, but a specific shift is not guarantee).
Under general supervision, monitors and support individuals diagnosed with an Intellectual developmental disability and/or co-occurring mental health and/or substance use disorder in living a meaningful and inclusive life in the community focusing on areas of Holistic Health, Safety, Rights, Choice, Community Integration and Achieving Results. Assist Individuals with hygiene care, meal preparation. Supervise self-administration of medications and document daily progress of service delivery.
Observes, monitors, documents and reports resident behavior/progress on an ongoing basis. Trains and assists residents in basic home maintenance skills, basic parenting skills, interpersonal development, and money management.
Maintains a clean, healthy and safe environment.
Provides transportation for residents to appointments and activities.
Performs community integration functions.
Candidates must have completed High school diploma or GED, be 21 years old or older to apply. CNA, PCA or DSP certification preferred.
One year experience providing social or direct care services is preferable.
Georgia Driver's License Required for this job (MVR is checked). You cannot have more than 2 moving violations in the past 3 years or a suspended license or DUI in the past 5 years.
All selected applicants will be subject to the following prior to employment: MVR, Drug Screen, National (NCIC) and Local (GCIC) Background Check.
