About the RN MDS Coordinator Role:
As a RN Registered Nurse MDS Coordinator, you will play a critical role in the clinical documentation and assessment process. Your responsibilities will include:
Collaborating with our interdisciplinary care team to ensure accurate and timely completion of Minimum Data Set (MDS) assessments and care plans
Conducting resident interviews and gathering information to inform MDS assessments and care plans
Maintaining accurate and up-to-date records of resident assessments and care plans
Providing education and support to our care team members on MDS-related documentation and assessment processes
Participating in quality improvement initiatives to ensure effective and efficient MDS processes
RN MDS Coordinator Requirements:
Current licensure as a Registered Nurse in the State of New York
Prioi experience in a long-term care setting, preferably as an MDS Coordinator
Strong communication and interpersonal skills to effectively collaborate with the care team
Proficiency in electronic medical records and MDS software
Ability to maintain confidentiality and handle sensitive information
