Responsibilities
- Deliver compassionate, patient-centered care by assisting with daily activities such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and patient transfer methods.
- Monitor vital signs and observe changes in patients’ conditions, reporting findings accurately in medical documentation systems such as EMR (Electronic Medical Records) and EHR (Electronic Health Records).
- Administer medications and perform basic clinical tasks following strict aseptic techniques and safety protocols to ensure patient safety and compliance with HIPAA regulations.
- Support patients with specialized needs including dementia, Alzheimer’s, behavioral health challenges, or developmental disabilities through tailored care plans.
- Collaborate effectively with interdisciplinary teams in diverse settings such as ICU (Intensive Care Unit), emergency medicine units, post-acute care facilities, or outpatient clinics to ensure seamless patient care.
- Maintain patient confidentiality diligently while managing medical records and documentation with precision across various EMR/EHR platforms like Cerner or eClinicalWorks.
- Assist with patient transfers using safe handling practices while adhering to infection control standards for infectious disease care or trauma situations.
Requirements
- Valid CNA certification or equivalent licensure required; additional certifications such as CPR/BLS are highly valued.
- Experience working with geriatric patients, individuals with disabilities, or in specialized environments like hospice, ICU, PICU (Pediatric Intensive Care Unit), or Level I/II Trauma Centers is preferred.
- Demonstrated knowledge of medical terminology, anatomy, physiology, and medication administration procedures.
- Proficiency in using EMR/EHR systems for accurate documentation and patient record management.
- Ability to perform heavy lifting safely and execute manual handling techniques essential for patient transfers and mobility assistance.
- Strong communication skills to effectively engage with patients, families, and healthcare teams while maintaining professionalism at all times.
- Commitment to maintaining patient confidentiality under HIPAA guidelines and practicing infection control protocols diligently.
