Responsibilities
- Lead the development and completion of Minimum Data Set (MDS) assessments in accordance with federal regulations and facility policies to support patient care planning and reimbursement processes.
- Oversee clinical documentation review, ensuring accuracy, completeness, and compliance with HIPAA, NCQA standards, and other regulatory requirements.
- Coordinate discharge planning, hospice care referrals, and transition of care to promote seamless patient experiences across inpatient, outpatient, home care, and emergency settings.
- Utilize EMR (Electronic Medical Record) systems such as Cerner, Epic, Athenahealth, or eClinicalWorks to document patient information efficiently and accurately.
- Conduct utilization review and management activities to optimize resource use while maintaining high-quality care standards for acute care, ICU, PICU, trauma centers, and other specialized units.
- Collaborate with physicians, case managers, coding specialists, and other healthcare professionals to ensure precise ICD-10/ICD-9/ICD coding and CPT coding for billing accuracy.
- Support clinical documentation improvement initiatives by analyzing medical records for completeness and providing education on medical terminology, anatomy knowledge, physiology concepts, and medical records management.
Qualifications
- Valid Registered Nurse (RN) license with current state licensure.
- Proven experience in MDS coordination within skilled nursing facilities or long-term care environments.
- Strong knowledge of ICD coding (ICD-10/ICD-9), CPT coding, DRG assignment, Medicare regulations, and utilization management processes.
- Experience working with EMR/EHR systems such as Cerner, Epic, Athenahealth or similar platforms.
- Background in managed care settings or hospital environments including ICU, trauma centers (Level I or Level II), or emergency medicine is highly desirable.
- Familiarity with clinical documentation improvement strategies and health information management standards.
- Excellent understanding of medical terminology, anatomy, physiology concepts related to inpatient/outpatient care settings.
- Ability to navigate complex medical records while ensuring compliance with HIPAA regulations.
