Position Summary
The Licensed Physical Therapist is responsible for evaluating and treating patients, collaborating with interdisciplinary teams, and providing high-quality rehabilitative services. This role also includes maintaining accurate documentation and participating in staff training and development related to restorative techniques.
Key Responsibilities
Patient Care & Evaluation

Evaluate patients based on physician referrals/orders

Develop personalized treatment plans with goal-driven outcomes

Provide one-on-one therapeutic interventions

Participate in discharge planning and patient education
Documentation & Compliance

Maintain timely and accurate documentation in accordance with professional and regulatory standards

Record daily treatment charges following approved billing guidelines

Stay current with Medicare and other reimbursement regulations
Team Collaboration

Communicate effectively with patients, families, and interdisciplinary team members

Participate in team meetings, care conferences, and performance improvement programs

Coordinate services across departments to support comprehensive care
Leadership & Supervision

Supervise Physical Therapist Assistants, rehab techs, and therapy students

Conduct in-service training for staff in restorative techniques

Provide mentorship and support to new team members
Facility Support

Maintain cleanliness and organization in treatment areas

Ensure equipment is functioning properly and report any maintenance needs

Travel or float to other facilities as needed based on patient care demands
Exposure Risk
This role is considered high risk for exposure to blood and bodily fluids.
Supervision Structure

Reports to: Director of Rehab or Designee

Supervises: Therapy Assistants, Rehab Techs, Students, and Provisional Licensees
Working Conditions

Active environment with intermittent sitting, standing, lifting, and moving

Involvement with patients and families under various emotional conditions

Must be alert to ensure patient safety at all times

Willingness to work flexible hours as needed

Potential exposure to infectious diseases and various physical hazards
Qualifications
Education & Licensure

Graduate of an accredited Physical Therapy program

Valid state licensure or eligibility for licensure in the state of practice
Physical Requirements

Ability to lift up to 50 lbs and assist with patient mobility

Capable of walking, stooping, bending, pushing, and pulling throughout the workday

Strong communication skills and emotional resilience

Ability to work independently and as part of a team in a fast-paced environment
