Compensation
- LVN: $72,000–$78,000 annually
- RN: $82,000–$100,000 annually
- Compensation is based on experience and qualifications.
- Coordinate and complete accurate MDS assessments within required timeframes.
- Collect, review, and verify resident clinical information.
- Ensure MDS documentation complies with federal, state, and facility requirements.
- Monitor assessment schedules and maintain timely completion of all required documentation.
- Collaborate with nursing, therapy, social services, dietary, and other interdisciplinary team members.
- Participate in care plan meetings and assist with the development of individualized resident care plans.
- Review medical records to ensure documentation supports resident needs, services, and reimbursement.
- Identify documentation concerns and provide education to clinical staff as needed.
- Maintain the confidentiality of all resident and facility information.
- Support quality assurance and performance improvement initiatives.
- Current and active California LVN or RN license.
- Previous experience in skilled nursing, long-term care, or post-acute care preferred.
- MDS and care planning experience preferred.
- Knowledge of Medicare, Medicaid, PDPM, and regulatory requirements is a plus.
- Strong organizational, communication, and time-management skills.
- Ability to work effectively with an interdisciplinary healthcare team.
- Excellent attention to detail and commitment to accurate documentation.
