The Pike County Health Department is seeking a Diabetes Prevention Coordinator to lead and support diabetes prevention efforts in the community. This position focuses on education, outreach, participant support, program coordination, and community partnerships.
The coordinator will work with individuals, healthcare providers, and community organizations to promote diabetes prevention, encourage healthy lifestyle changes, and support the overall planning and evaluation of the program.
Responsibilities
- Provide diabetes prevention education and support to program participants
- Coordinate program activities, outreach, and follow-up
- Educate participants on diabetes risk factors, prevention strategies, and healthy lifestyle habits
- Build and maintain relationships with healthcare providers, community organizations, and referral partners
- Assist with participant recruitment and community outreach efforts
- Maintain accurate program records and documentation
- Track participation and program outcomes
- Assist with program planning, evaluation, and continuous improvement
- Help develop program materials, policies, and procedures as needed
- Participate in community events, presentations, and other diabetes prevention activities
- Perform other related duties as assigned
Requirements
- Associate Degree in Nursing
- Active Registered Nurse license in Kentucky or a compact state
- Minimum of 2 years of relevant professional experience
- Eligibility for Diabetes Educator Certification
Pay: $28.00 - $32.00 per hour
Expected hours: 37.5 per week
