PIH Health is seeking a dedicated Nurse Practitioner (APCNP) to join its Post-Acute Care team in Whittier, California. This role focuses on providing high-quality, evidence-based care for patients transitioning from hospital to home, with an emphasis on older adult and medically complex populations.
The APC Nurse Practitioner functions as a clinical expert in an advanced practice role, independently delivering comprehensive, patient-centered care across inpatient follow-up, outpatient clinic, and home-based settings.

Required Skills And Experience
- Current California Nurse Practitioner License
- Master’s degree in nursing from an accredited nurse practitioner program
- Board Certification: Board certified as a Nurse Practitioner
- Two to five (2-5) years previous experience in chronic care/palliative care outpatient setting
- Organizational skills and personal leadership qualities including tact and diplomacy, necessary to coordinate all aspects of the program and to integrate the efforts of the hospital, medical staff, and program staff
- Evidence of continuing education and self-development to support license requirements
- Current Basic Life Support Card
- California Driver’s License and current car insurance required
- Bilingual skills in Spanish/Chinese preferred.

Salary : $130,000 - $160,000
