Case Manager Registered Nurse (RN) or Licensed Vocational Nurse (LVN) Full-Time, 8:30 AM - 5:00 PM

The RN or LVN Care Manager participates in a continuum of care from pre-admission through post-discharge for assigned patients.  Assure that patients and families proceed efficiently throughout the course of hospitalization.  Assists in the appropriate utilization of hospital services, maximizing reimbursement from all third-party payors, and education of medical/hospital staff about healthcare options.

Salary range varies by license:

RN: $55.00 - $87.50/hour  

LVN: $33.35 - $55.75/hour

Required Skills

• Broad knowledge of disease process, pathophysiology, and treatment plan
• Well-honed organizational skills and familiarity with medical assessment
• Familiarity with the discharge process and care options

Required Experience

Required:

• Maintain an active California RN or LVN license (as applicable).
• Computer proficiency
• Minimum of 3 years clinical nursing experience. 

Preferred:

• Previous experience in case management preferred.
• Understands treatment plans and healthcare delivery system.

Salary : $33 - $56
