Qualifications
Education
- Master's Degree in Social Work from an accredited social work program Required
- 1 year of experience in an acute or post-acute setting Required
- None Required
- LMSW - Licensed Masters Social Work Preferred
- Accredited Case Manager (ACM) Preferred
- CCM - Certified Case Manager Preferred
