Job Title – Physical Therapist
Department - PACE/Senior Care

- Work Location: 163 Chatham Business Dr, Pittsboro, NC 27312
- Schedule: Monday through Friday, 8:00am – 5:00pm
- Patient Population: Adult Geriatric
- Conducts an initial assessment and scheduled reassessment of all participants; communicates participant changes to team members.
- Develops treatment program when indicated, sets goals and provides treatment.
- Reviews and completes assessments and discharge summaries. Plans and develops maintenance, exercise and therapy programs for participants.
- Provides manual muscle testing; sensory/perceptual testing; neuromotor functioning; appropriateness of current assistive/adaptive/orthotic devices.
- Participates as a member of IDT; attends staff meetings and takes part in participant care planning; communicates any changes in participant condition to team members.
- Must be able to demonstrate clinical competency with geriatric population
- Must be able to work under pressure
- Must meet a standardized set of competencies for the specific position description established by PHSC and approved by CMS before working independently.
- Excellent interpersonal, oral and written communication skills
