Role Summary:
We seek an experienced Residential Coordinator with the primary responsible for assisting Residential Director with development & operations of all CTH facilities
Key Responsibilities:
- Monitor and record patient vital signs and health status.
- Assist in the development and implementation of patient care plans.
- Conduct patient assessments and document findings accurately in medical records.
- Collaborate with healthcare team members to coordinate patient care.
- Maintain a clean and safe environment for patients.
- Assist with diagnostic tests and procedures as needed.
- Ensure compliance with HIPAA regulations regarding patient confidentiality.
Requirements
- Valid LPN license in the state of practice.
- Experience is preferred.
- Strong skills in patient assessment, medication administration, and vital sign monitoring.
- Experience working with diverse populations including those with developmental disabilities, Alzheimer’s disease, dementia care, or geriatrics is beneficial.
- Excellent communication skills to effectively interact with patients, families, and healthcare team members.

Base Pay: $23.00 - $25.00 per hour
