Physician Services Group (PSG) is seeking a Part-Time Nurse Practitioner (NP) to join our Post-Acute Care team in Estill, South Carolina. This role focuses on delivering high-quality, patient-centered care within Skilled Nursing and Post-Acute Care facilities—helping reduce hospital readmissions while building long-term relationships with patients and care teams.
This is a weekday-only position (2 days per week) offering schedule consistency, clinical autonomy, and strong physician collaboration.
Schedule & Work Environment
- Part-Time | Monday–Friday
- Typical hours: 8:00 AM – 4:00 PM
- On-site at a Post-Acute / Skilled Nursing Facility
- No nights/ weekends,
Key Responsibilities
- Evaluate and manage new admissions, follow-ups, and long-term care patients
- Provide chronic disease management and preventative care
- Collaborate closely with attending physicians and facility staff
- Deliver care in accordance with PSG clinical guidelines
- Maintain accurate and timely documentation
- Serve as the primary on-site provider representing PSG
Qualifications
- Active South Carolina Nurse Practitioner license
- Board Certified (ANCC, AANP, or equivalent)
- Strong clinical decision-making and documentation skills
- Experience in post-acute, hospital, SNF, or outpatient settings preferred
- Interest in geriatrics and continuity of care strongly valued
