Physician Services Group (PSG) is expanding our Post-Acute Care team and is seeking a Nurse Practitioner/ Physician's Assistant to support Skilled Nursing and Post-Acute facilities in the Georgetown, South Carolina area.
This position is ideal for an NP seeking work-life balance, schedule consistency, and clinical autonomy while building long-term patient relationships.
Schedule & Work Environment
- Part-Time | 3 days per week (Monday–Friday)
- Typical hours: 8:00 AM – 4:00 PM
- On-site at Skilled Nursing / Post-Acute facilities
- No nights, weekends, or on-call
Key Responsibilities
- Evaluate and manage new admissions, follow-ups, and long-term care patients
- Provide chronic disease management and preventative care
- Collaborate with attending physicians and facility care teams
- Maintain accurate and timely clinical documentation
- Serve as PSG’s on-site clinical representative
Qualifications
- Active South Carolina Nurse Practitioner license
- Board Certified (ANCC, AANP, or equivalent)
- Strong clinical judgment and documentation skills
- Experience in post-acute, hospital, SNF, or outpatient settings preferred
- Interest in geriatrics and continuity of care strongly valued
Job Type: Part-time
Pay: $55.00 - $65.00 per hour
