Physician (MD/DO) – Post-Acute Care | Jacksonville/ St. Augustine locations
Physician Services Group (PSG) is expanding and seeking a dedicated Physician to join our growing Post-Acute Care team.
Enjoy a weekday-only schedule, caring for patients in Skilled Nursing, Assisted Living, and Rehab settings.
Responsibilities
- Provide direct patient care: admissions, follow-ups, and scheduled Medicare visits.
- Manage long-term and acute treatment plans for a stable post-acute population.
- Lead and support Nurse Practitioners and Medical Assistants on-site.
- Collaborate with facility teams to deliver comprehensive, quality care.
- Document visits and billing through EMR with appropriate coding.
Qualifications
- MD or DO (Board Certified/Eligible in a Primary Care specialty).
- Active Florida medical license and DEA.
- Strong knowledge of chronic and post-acute care.
- EMR and coding experience preferred.
- Leadership skills and ability to work independently.
Schedule
- Full-time: Monday–Friday, 5 days/week.
- 6–8 hour shifts, ~16 patients/day.
- No nights / weekends
