This role establishes individualized physical therapy goals, provides treatments, and regularly reassesses patient progress, documenting all findings accurately.
Essential Functions
- Conducts thorough physical therapy evaluations to assess patient needs and develops individualized treatment plans.
- Administers safe and effective therapy treatments, ensuring all interventions are appropriately documented.
- Reassesses and adjusts treatment plans within appropriate timeframes to align with patient progress and changing needs.
- Provides clear, concise, and accurate documentation of evaluations, treatment plans, progress notes, and billing information.
- Educates patients and their families on treatment plans, therapeutic exercises, and post-treatment care to support long-term recovery.
- Operates therapy equipment safely and maintains a clean, organized, and safe environment for patients and staff.
- Collaborates with referring physicians, peers, and interdisciplinary team members to ensure continuity and quality of patient care.
- Demonstrates population-appropriate competencies for all relevant patient demographics, tailoring care to individual needs.
- Maintains compliance with facility policies, including safety, infection control, and risk management procedures.
- Adapts to changes professionally and effectively, following the chain of command to address and resolve issues impacting patient care.
- Performs other duties as assigned.
- Maintains regular and reliable attendance.
- Complies with all policies and standards.
- Doctorate in Physical Therapy (DPT) required
- 0-2 years of experience in a physical therapy setting required
- Strong knowledge of physical therapy principles, therapeutic techniques, and safety standards.
- Ability to perform comprehensive evaluations and develop effective treatment plans.
- Proficiency in using therapy equipment and technology for documentation and billing.
- Excellent communication and interpersonal skills to collaborate with patients, families, and interdisciplinary teams.
- Strong organizational and time management skills to prioritize tasks and maximize productivity.
- Commitment to professional development and adherence to ethical and legal standards.
- PT - Physical Therapist license in the state of employment required
- BCLS - Basic Life Support within 30 days of hire required
