Partnership Health Center is seeking a Part-Time Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to provide high-quality behavioral health services to employees of Somerset Township and their families. This is an excellent opportunity to work in a supportive, integrated primary care setting with a stable patient population.
Key Responsibilities
- Provide individual behavioral health counseling and short-term therapy
- Conduct comprehensive psychosocial assessments and develop treatment plans
- Deliver evidence-based therapeutic interventions
- Collaborate with primary care providers to ensure integrated care
- Maintain accurate and timely clinical documentation in the EMR
- Provide crisis intervention and risk assessments as needed
- Offer referrals to community resources when appropriate
- Participate in interdisciplinary team meetings
Qualifications
- Active New Jersey LCSW or LPC license (required)
- Experience in outpatient or integrated care settings preferred
- Strong clinical assessment and treatment planning skills
- Ability to work independently and as part of a multidisciplinary team
- Excellent communication and documentation skills
Schedule
- Part-time hours (flexible scheduling available)
Salary : $45 - $50
