APRN or Physician Assistant for Skilled Nursing Facilities - PC WH - East Hartford

Salary Range: $110,000.00 to $200,000.00 a year with quarterly bonus opportunity.By adhering to Connecticut State Law, pay ranges are posted. This is an excellent opportunity for those that enjoy a flexible work schedule, working autonomously, and accelerate in a pay for performance environment.The Nurse Practitioner or Physician Assistant will travel to Skilled Nursing Facilities contracted with PACT, LLC.Essential FunctionProvide medical care to a complex patient population in the short term rehab and long term care settingAnalyze patient’s medical history, medication allergies, physical condition and examination to determine best care for acute and chronic medical conditionsOrder, interpret, and evaluate diagnostic tests to identify and assess patient’s clinical problems and health care needsPrescribe pharmaceuticals, other medications, and treatment regimens as appropriate for assessed medical conditionsRecord in medical record the physical findings, formulated plans, and prognosis based on patient’s condition within appropriate time frameCoordinate with the nursing facility care planning team in the development of comprehensive care planCommunicate with patients and family members regarding admission, care plan, discharge needs, prognosis/end of life care as neededFollows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standardsEducation and ExperienceAPRNThe Advanced Practice Registered Nurse is masters or doctorate preparedCertified Nationally as a Nurse Practitioner through AANP or ANCC .The Nurse Practitioner must have possession of a valid (Connecticut) license as a Registered Nurse and an Advanced Practice Registered Nurse, and be certified as a y) Nurse Practitioner by an approved certifying body.DEA, and Ct controlled substance licenseExperience working with elderly patient populationExperience working with EHRPAPhysician Assistant CertificationState LicenseAny years of experienceExperience working with EHR#APRNCT #NursePractitioner #AdvancedPracticeRegisteredNurse #APRN #PA #PhysicianAssistant
