Physician Affiliate Group of New York (PAGNY) has an opportunity for a Program Assistant - Health Equity for Adolescents in the Legal System (HEAL) with NYC Health Hospitals/Administration for Children's Services. PAGNY staffs several municipal hospitals and clinics within New York City through an affiliation agreement with NYC Health Hospitals. PAGNY’s corporate staff provides administrative, practice management, and human resources services in support of its more than 4,000 clinical providers.
The HEAL Program is a new NYC Health Hospitals initiative to transition into being the primary provider of medical services to young people in City custody. The staff will be responsible for 24/7 medical care across two secure detention sites, two school-based health centers, and limited pharmacy services at seven non-secure sites (group homes). Services include comprehensive medical screenings, dental screenings and procedures, vaccinations and routine testing, responses to immediate need, and referrals for offsite care within and outside the NYC Health Hospitals system.

Opportunity Details
- Work locations = Crossroads Juvenile Center and Horizon Juvenile Center.
- Schedule = 37.5 hours per week - Must be flexible to cover call outs and vacations.
- Answer telephones and direct calls, as needed.
- Coordinate movement of charts and medication with medical staff and driver, as required.
- Log in all court orders and ensure appropriate tracking and compliance.
- Interface with ACS staff on specific questions and refer all ACS staff inquiries to the Deputy Director.
- Understand and use all essential electronic system functions to extract data from relevant systems for reports.
- Collect all sick call requests and schedule, as appropriate.
- Follow shift opening and closing procedures.

- Compile data and update Special Needs, Restricted Activities, and Diet Order forms daily by 8:30am and distribute to appropriate entities for Secure Detention and Non-Secure Detention Sites.
- Track house pass patients and schedule for referral medical visits.
- Update weekly specialty clinic schedules and coordinates movement of patients to these visits, plus track completed clinic visit.
- May be assigned to any site.
