Job Overview
We are seeking a dedicated and skilled Licensed Physical Therapist to join our healthcare team in a full-time or part-time capacity. The ideal candidate will possess a strong foundation in physiotherapy practices, with expertise in patient care across diverse settings including inpatient, outpatient, home health, and hospital environments. The successful applicant will demonstrate proficiency in medical terminology, anatomy, physiology, and the use of electronic medical records (EMR) systems.
Duties
- Conduct thorough assessments of patients’ physical conditions to establish accurate diagnoses and treatment goals.
- Develop and implement individualized care plans tailored to each patient’s needs, including discharge planning and ongoing progress evaluation.
- Provide hands-on physiotherapy interventions such as trigger point therapy, orthopedics, geriatrics, pediatrics, and community care services.
- Collaborate with multidisciplinary teams to coordinate patient care across inpatient, outpatient, home health, and hospital settings.
- Educate patients and their families on treatment techniques, injury prevention strategies, and home exercise programs to promote recovery and independence.
- Maintain detailed documentation of patient progress using EMR systems while ensuring compliance with healthcare regulations.
- Assist in managing acute care cases by applying physiotherapy techniques suited for hospital environments and complex medical conditions.
- Participate in team meetings to review treatment plans, share insights on patient progress, and contribute to quality improvement initiatives.
Experience
- Valid licensure as a Physical Therapist with relevant state or national certification.
- Proven experience working in inpatient hospital settings with exposure to acute care environments.
- Demonstrated expertise in outpatient clinics, home & community care, or home health services.
- Strong knowledge of anatomy, physiology, medical terminology, and therapeutic modalities such as trigger point therapy.
- Experience developing care plans for diverse populations including geriatrics and pediatrics.
- Familiarity with EMR systems used for documenting patient information accurately and efficiently.
- Prior involvement in discharge planning processes ensuring seamless transitions from hospital to home or community settings.
