Job Overview
We are seeking a dedicated and skilled Pharmacist to join our healthcare team. The position is Per-Diem / PT for flex hours. The ideal candidate will possess extensive clinical pharmacy experience, a strong commitment to patient-centered care. Compounding experience is preferred yet not required. This role involves providing expert medication management, compounding, patient assessment, and ensuring the safe and effective use of pharmaceuticals across diverse clinical environments. The Pharmacist will play a vital role in medication dispensing, compounding, and collaborating with healthcare professionals to optimize patient outcomes. Candidates with a background in hospital pharmacy, immunizations, and medical management are highly preferred. This position offers an opportunity to work in a dynamic healthcare setting with a focus on comprehensive patient care and medication safety.
Duties
- Dispense medications accurately using pharmacy software and EMR/EHR systems while adhering to HIPAA regulations
- Conduct thorough patient assessments to determine appropriate medication therapy and identify potential drug interactions
- Prepare and compound medications, including hazardous preparations using appropriate technique.
- Education on medication use, side effects, and health management strategies
- Manage inventory levels, order supplies, and ensure proper storage of pharmaceuticals in compliance with safety standards
- Collaborate with healthcare providers on medical management plans, pain management protocols, and medication adjustments
- Document all patient interactions, medication dispensing activities, and clinical notes accurately within electronic systems
- Provide clinical pharmacy services in specialized areas such as pediatrics, acute care, or relatable fields.
- Participate in patient care initiatives including medication administration oversight and patient service programs
- Support pharmacy technician staff by overseeing workflow, verifying prescriptions, and ensuring quality control
Skills
- Clinical pharmacy experience with expertise in compounding medications.
- Knowledge of physiology, anatomy, medical terminology, and pharmacology to inform patient assessments and treatment plans
- Proficiency with pharmacy software systems as well as EMR/EHR platforms for documentation and medication management
- Experience with immunizations administration and adherence to HIPAA regulations for patient privacy
- Strong skills in patient service delivery, including medication counseling and health education
- Familiarity with inventory management practices to ensure optimal stock levels and compliance with safety standards
- Ability to work effectively within multidisciplinary teams on medical management initiatives such as pain management or pediatrics
- Excellent communication skills coupled with the ability to assess patients' needs accurately for optimal therapeutic outcomes
