Part-Time Case Manager (with potential to move to full-time)

Phoenix Transitional Housing is seeking a dedicated and compassionate Part-Time Case Manager to join our Visalia team. This position supports young adults (ages 18–21) transitioning out of foster care or probation by providing guidance, resources, and accountability to help them achieve independence.
Key Responsibilities
Provide individualized case management services to assigned clients
Conduct regular home visits and maintain consistent client contact.
Assist clients with employment, education, and life skills development
Support clients in accessing community resources (mental health, medical, education, etc.)
Maintain accurate and timely documentation (case notes within 24 hours, compliance records)
Participate in Child and Family Team (CFT) meetings and collaborate with social workers, probation officers, and service providers
Enforce program policies and provide accountability while maintaining a trauma-informed approach
Qualifications
Bachelor’s degree in Behavioral Science (preferred) or minimum of 2 years of experience working with at-risk youth or young adults
Strong organizational and documentation skills
Ability to manage time effectively and work independently
Knowledge of trauma-informed care and strength-based practices
Valid CA driver’s license and reliable transportation
This position requires livescan clearance and a clear TB test.
Pay: $25.00 - $30.00 per hour
