Phoenix Home Care & Hospice is seeking an experienced and compassionate On-Call Registered Nurse to support our Topeka hospice team. This role is essential to delivering timely, skilled, and meaningful end-of-life care in patients’ homes—exactly when they and their families need it most. What You’ll Do
- Respond to the immediate care needs of hospice patients and families during nights and weekends
- Complete routine and as-needed visits in home settings across the Topeka region
- Triage urgent situations and collaborate with hospice physicians to provide comfort and symptom management
- Work closely with the interdisciplinary hospice team, community partners, and family caregivers
- Maintain accurate clinical documentation and uphold all regulatory and agency standards
- Deliver professional, compassionate, patient-centered hospice care
- Participate in ongoing training, education, and quality initiatives
What You Bring
- Kansas RN License or Compact License
- Hospice nursing experience preferred (Topeka hospice experience is a plus)
- Strong clinical judgment with the ability to work independently in home settings
- Excellent communication and customer service skills
- Valid driver’s license and reliable transportation
- High-level organization, time management, and emotional resilience
