SUMMARY: As a Case Manager, you will play a pivotal role in coordinating and facilitating comprehensive care for individuals dealing with substance use disorders. Your expertise in treatment planning, resource coordination, and ongoing support will contribute to the successful recovery and well-being of our clients.
Responsibilities:
- Conduct thorough assessments of clients' substance use history, mental health, and psychosocial needs to inform personalized treatment plans.
- Collaborate with clients to set achievable recovery goals and develop individualized care plans that address their unique challenges and aspirations.
- Coordinate and advocate for a continuum of care, including medical, mental health, and social services, to ensure holistic support for clients' recovery journey.
- Monitor clients' progress and regularly reassess treatment plans, making adjustments as needed to promote successful outcomes.
- Provide crisis intervention and support to clients in acute situations, linking them to appropriate resources and services.
- Facilitate group and individual therapy sessions, using evidence-based approaches to address addiction, relapse prevention, coping skills, and life management.
- Connect clients with community-based resources, support groups, employment assistance, housing services, and educational opportunities.
- Maintain accurate and up-to-date client records, documenting interventions, progress, and referrals in compliance with ethical and legal standards.
- Collaborate with a multidisciplinary team, including medical professionals, counselors, social workers, and support staff, to provide integrated care.
- Communicate effectively with clients' families, legal representatives, and community partners to ensure a comprehensive and coordinated approach to care.

Qualifications:
- Bachelor's or Master's degree in Social Work, Psychology, Counseling, or a related field from an accredited institution.
- CDCA Required.
- Proven experience in case management within substance use disorder treatment settings, with a strong understanding of addiction dynamics, treatment modalities, and community resources.
- Excellent communication and interpersonal skills, with the ability to build rapport and establish effective relationships with clients and colleagues.
- Knowledge of legal and ethical standards governing substance use disorder treatment and case management.
- Strong organizational skills and attention to detail in managing client documentation and records.
- Ability to work collaboratively within a team and adapt to changing client needs and treatment approaches.
- Proficiency in crisis intervention and problem-solving in high-stress situations.
