Phoenix Community Development Services mission of “Turning Lives Around” reflects our long history of serving populations of individuals who have limited resources and support systems, including those have been homeless or who are currently facing homelessness who have mental health challenges and face financial burdens such as extremely low to zero income. Phoenix CDS is committed to providing comprehensive, supportive services to our most vulnerable citizens who face great challenges. Our primary goal is to end homelessness and provide the individuals, families and veterans we serve with a warm, safe place to live independently – and away from the dangers of the streets. Purpose of the Position

Join our dedicated team as a Case Manager specializing in Permanent Supportive Housing, where your passion for social impact and commitment to empowering vulnerable populations will shine. In this vital role, you will coordinate comprehensive support services for individuals housing instability, helping them achieve stability and independence. Your proactive approach and empathetic communication will foster trusting relationships, ensuring clients receive tailored assistance that promotes long-term success. This position offers an exciting opportunity to make a meaningful difference in the community while working within a dynamic, supportive environment.

Qualifications/Skills/Competencies

  • Current and Valid Driver's License
  • Must have a High School Diploma or GED
  • Preferred experience in a field related to mental health supportive services and/or substance use disorder
  • Knowledge of various clinical criteria related to issues confronting individuals served (i.e. mental health, substance use, housing)
  • Ability to perform crisis intervention and crisis management techniques confidently under pressure.
  • Excellent communication skills with an empathetic approach to client engagement through motivational interviewing techniques.
  • Use of Rules of Professional Ethics, Regulations and Confidentiality and apply to daily work situations

Knowledge of Windows Office Systems including Outlook, Word, and Excel

Work Conditions

  • The case manager has a designated office but is expected to spend the majority of the day in the community serving clients
  • Expect to interact with staff and clients to provide activities related to work such as conducting meetings, groups, and/or workshops
  • May require prolonged sitting or standing
  • May require lifting above 50lbs on occasion to assist individuals served with acquiring furniture, housing relocation, etc.
  • Requires driving throughout community during course of normal business

Pay: $17.00 - $20.00 per hour
