Title: Outreach Coordinator
Location: Crotona Park West
Reports to: Program Director
Status: Tuesday-Friday 9am-5pm
Program Description:
The Outreach Specialist will be conducting outreach to the residents of Phipps Properties to connect residents to services offered by Phipps Neighborhoods. The Outreach Specialist will circulate flyers, host workshops, and meetings with residents who are 21 & up. The Outreach Specialist will provide through the Resident Services Specialist referrals to: Financial Counseling, HRA for One Shot Deals, Screening for Food Stamps, Enrollment for Medical Insurance, Education and Career programs such as: HSE, Vocational Training, Employment, and Internships. As well as: Substance Abuse Treatment, and Mental Health Services.
Responsibilities:
- Cultivate and maintain relationships with residents of all of Phipps properties in order to connect them with services.
- Actively seek presentation sites for health education such as childcare and health care service settings, public and private schools, community centers, parenting programs, housing programs and other relevant organizations.
- Participate in supervision, staff, and division meetings. Track data in salesforce data base system.
- Design activities to recruit residents to participate in any opportunities available.
- Oversee systems to document efforts, client participation, referrals, and follow-up.
- Introduce best practices, pilot activities, and adopt innovative approaches to recruiting and maintaining a group of community stakeholders.
- Participate in relevant Maternal/Child Health (MCH) Learning Communities, advisory meetings and health coalitions.
- Assist in the creation and distribution of program marketing materials including flyers, fact sheets, program information, reports, and a programmatic newsletter to targeted audiences.
- Participate in data collection, program evaluation, and activity/event tracking for program and contract reporting.
- Coordinate data collection and maintain a database that demonstrates integration with residents and partner agency. .
- Provide and present data relative to those in need of services, such as day care centers, health programs, community-based organizations, food pantries, health fairs and community events.
- Pursue quality assurance and improvement on a continuous basis.
- Other task deemed necessary by the Director or the Deputy Executive Director.

Qualifications:
- High School Diploma, or GED required, with outreach experience.
- Previous experience in the following areas: mental illness, substance abuse, housing, health, entitlements, workforce, and formerly incarcerated.
- Qualified candidate must have a strong understanding of the developmental needs of young adults, sensitivity to ethnic and cultural diversity, and knowledge of and commitment to working from a strength-based perspective.
- Applicant must have excellent written and verbal communication skills, as well as strong computer skills.
- Bi-lingual (Spanish) a plus.
