Job Summary
Join our dedicated healthcare team as a Private Duty Nurse (LPN or RN), where you will provide personalized, one-on-one nursing care to patients in their homes or private settings. Responsibilities
- Deliver comprehensive nursing care to patients in private home settings, assisted living facilities, or other community environments.
- Perform detailed patient assessments, monitor vital signs, and document medical information accurately using electronic health record (EHR) systems such as Epic or Cerner.
- Collaborate with physicians and healthcare teams for diagnostic evaluations, medication adjustments, discharge planning, and case management.
- Provide education on health coaching topics like nutrition, medication administration, and disease management tailored to individual needs across diverse populations including pediatrics and geriatrics.
- Perform full assessments and vital signs monitoring
- Provide tube feeding care (including flushing, positioning, and aspiration precautions)
- Reposition patient every 2 hours to prevent skin breakdown
- Monitor for UTI, constipation, dehydration, and respiratory distress
- Educate and support caregiver on care plan and safety measures
- Report any changes in condition immediately
Skills Required:
- G-tube care and management
- Enteral feeding (bolus continuous overnight feeds)
- Medication administration via feeding tube
- Monitoring for respiratory, GI, and skin complications
- Full assistance with ADLs (bathing, dressing, diapering, hygiene)
- Ability to manage behavioral challenges in a low-stimulation environment
Experience
- Valid LPN or RN license with current state certification and compliance with HIPAA regulations.
- Extensive background working with individuals with disabilities, including developmental delays and behavioral health considerations.
- Hands-on experience in various settings such as hospital medicine, post-acute care facilities, nursing homes, hospice care, home & community care environments, or assisted living communities.
- Knowledge of ICD coding (ICD-9/10), CPT coding for procedures and documentation standards related to medical-surgical and emergency medicine practices.
- Skills in advanced patient assessment techniques including vital signs monitoring, physiological evaluations, and diagnostic evaluation procedures.
- Familiarity with infection control protocols, sterile technique practices like suturing and spinal taps are advantageous.
