What are the responsibilities and job description for the Elementary School Social Workers position at Phelps-Clifton Springs Central School District?
The Phelps-Clifton Springs Central School District has an opening for an Elementary School Social Worker. The successful candidate will need to:
- Connecting students and families with school and community-based resources;
- Identify and promote internal use of community-based services directly and in a consultation role;
- Providing case management for students and families requiring multiple resources;
- Providing school-based counseling per IEP, 504 or student support plan;
- Promote social-emotional well-being of students in classrooms, lunch groups, restorative practices, etc.;
- Support the implementation of functional behavioral assessment and behavior intervention plans in collaboration with other mental health professionals;
- Respond as needed in crisis situations;
- Participate in IEP, 504 meetings, building team meetings;
- Serve as member of Pupil Support Office Team;
- Utilize data to monitor attendance, behavior, academics and collaborate with building team, families and students on appropriate interventions to ensure students remain on-track;
- Lead attendance monitoring, reporting, and outreach efforts;
- Collaborate with school counselors on fostering community partnerships in the region for career exploration.
Job Qualifications
- School Social Worker Certification required, MSW or CSW preferred
- Workshops – Child Abuse Identification (Second Update), School Violence Intervention and Prevention, Dignity for All Students Act
- Fingerprint Clearance
- Experience or familiarity with DSM V and Medicaid Billing
- Experience or familiarity with focused, research-based therapies and behavioral supports
