Responsibilities:

• Provide in-home physical therapy services to patients across the Tri-County area.
• Evaluate, plan, and implement personalized treatment plans.
• Collaborate with interdisciplinary team members to ensure optimal patient outcomes.
• Educate patients and families on care plans and safe practices.

Requirements:

• Current Physical Therapist (PT) license in South Carolina
• Prior experience in home health (preferred but not required)
• Reliable transportation and valid driver's license
• Strong communication and clinical skills
• Compassionate, self-motivated, and team-oriented
• Proficient and timely with EMR point of care documentation

Pay: $85,000.00 - $95,000.00 per year
