We are seeking an experienced Registered Nurse (RN) MDS Coordinator to join a dedicated skilled nursing team in Lockport, NY!

💼 Key Responsibilities:
Oversee accurate and timely MDS assessments and submissions.
Ensure compliance with federal and NYS regulations.
Lead interdisciplinary care planning and coordination.
Review clinical documentation and identify opportunities for improvement.
Collaborate with nursing leadership to support quality resident care and accurate reimbursement.
Participate in utilization review meetings and daily clinical huddles.
✅ Qualifications:
Active NYS RN license required.
