What You’ll Do:
- Conduct client interviews and assessments using risk assessment tools to determine eligibility and complete new client intakes.
- Partner with clients to create personalized service plans that support their short- and long-term goals.
- Connect individuals and families to vital resources such as housing programs, public benefits, employment services, and legal assistance.
- Work closely with landlords and clients to ensure housing meets habitability standards and help resolve tenancy issues.
- Assist clients with applying for benefits, entitlements, and emergency financial assistance to address urgent housing needs.
- Provide ongoing support through regular follow-ups and progress tracking.
- Maintain accurate, organized case documentation and ensure files remain audit-ready in both paper and electronic systems.
